Position Summary:
Join a model built for providers who want more than clinic-based care. As a Wound Care focused Nurse Practitioner , you will deliver hands-on, high-impact care directly inside senior communities, managing complex wounds while serving as a trusted clinical partner to staff and families. This role offers true autonomy, consistent patient relationships, and the ability to see the direct results of your care over time . You will be part of a team that values clinical excellence, responsiveness, and innovation in geriatric medicine . Wound care experience or certification is highly preferred.
Responsibilities:
• Provide on-site rounding care and advanced wound management within senior living communities.
• Perform routine and complex assessments, developing evidence-based treatment plans .
• Support continuity of care by following patients across different care settings as needed.
• Collaborate with community staff, physicians, and interdisciplinary teams to optimize outcomes.
• Deliver proactive care coordination , reducing ER visits and hospitalizations.
• Communicate clearly with families and caregivers , building long-term trust.
• Utilize technology and documentation tools to support efficient, high-quality care delivery .
Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota .
• Experience in geriatrics, primary care, or wound care preferred .
• Valid driver’s license and ability to travel between communities.
• Passion for relationship-based, patient-centered care in senior populations .
• Strong clinical judgment with the ability to work independently.
