Position Summary:
We are seeking a Wound Care focused Nurse Practitioner to provide mobile, on-site care across senior living communities . In this role, you will manage complex wounds, support chronic condition management, and serve as a key clinical partner within each community. This position offers high autonomy, flexible scheduling, and the ability to build long-term patient relationships while practicing at the top of your license.
Responsibilities:
• Provide on-site rounding, primary care, and wound care services .
• Manage advanced wound cases including pressure ulcers, diabetic wounds, and skin integrity issues .
• Drive preventative care and chronic disease management strategies .
• Collaborate with physicians, nursing staff, and interdisciplinary care teams .
• Reduce hospitalizations through early intervention and consistent follow-up .
• Communicate with families and caregivers to align on care plans .
• Maintain accurate documentation, medication management, and lab coordination .
Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred) .
• Experience in geriatrics, wound care, primary care, or post-acute settings preferred .
• Valid driver’s license and ability to travel between communities.
• Strong ability to work independently in a field-based environment .
• Passion for delivering high-quality care to the senior population .
